How to code Deep Vein Thrombosis

Lower extremity DVT is coded from I82.4- (acute) or I82.5- (chronic). The fifth character names the vein: 0 unspecified deep vein, 1 femoral, 2 iliac, 3 popliteal, 4 tibial, 5 peroneal, 6 calf muscular, 9 other specified, Y proximal unspecified, and Z distal unspecified. The sixth character is laterality: 1 right, 2 left, 3 bilateral, 9 unspecified. A DVT documented without acuity is coded as acute, so a right femoral DVT with no other detail is I82.411 and a DVT with no vein or side documented is I82.409.

Chronic codes (I82.5-) are for a thrombus the provider describes as chronic or long-standing, typically a patient still being treated months after diagnosis. Add Z79.01 for current anticoagulant use. Once the clot has resolved and the patient has no current treatment beyond prophylaxis, report Z86.718 (personal history of venous thrombosis) rather than a chronic code. I82.5- carries an Excludes1 note for Z86.718, so the two are not reported together.

Upper extremity thrombosis uses I82.6- (acute) and I82.7- (chronic), each split into superficial and deep veins, while the axillary (I82.A-), subclavian (I82.B-), and internal jugular (I82.C-) veins have their own subcategories. Superficial thrombophlebitis of the leg is I80.0-, not I82. In pregnancy or the puerperium, an O22.3- or O87.1 code is sequenced first.

When a thrombophilia is documented, add the specific code such as D68.51 (factor V Leiden) or D68.61 (antiphospholipid syndrome). A pulmonary embolism accompanying the DVT is coded separately from I26.
