How to code Congenital Heart Defects

Septal defects are in category Q21. Q21.0 is ventricular septal defect. Atrial septal defect is subdivided: Q21.10 unspecified, Q21.11 secundum, Q21.12 patent foramen ovale, Q21.13 coronary sinus, Q21.14 superior sinus venosus, Q21.15 inferior sinus venosus, Q21.16 sinus venosus unspecified, Q21.19 other. Atrioventricular septal defect is Q21.20 to Q21.23 (unspecified, partial, transitional, complete). Q21.3 tetralogy of Fallot is a combination code, so its component defects are not coded separately.

Valve and vessel malformations have their own categories: Q22 pulmonary and tricuspid valves (Q22.1 congenital pulmonary valve stenosis), Q23 aortic and mitral valves (Q23.4 hypoplastic left heart syndrome, Q23.81 bicuspid aortic valve), Q20 chambers and connections (Q20.3 transposition of the great arteries), Q25.0 patent ductus arteriosus, and Q25.1 coarctation of the aorta. Q24.9 is an unspecified congenital heart malformation. When several distinct defects are documented, code each one.

A repaired defect that is no longer present is reported with Z87.74 (personal history of corrected congenital malformations of the heart and circulatory system); a defect that persists or has residual effects keeps its Q code at any age. A patent ductus arteriosus in a premature infant is still Q25.0, with the P07 codes added. On the birth admission Z38 is principal and the Q code is secondary.
