How to code Cirrhosis of the Liver
K74.60 is unspecified cirrhosis of liver and is the default when the cause is not documented. K74.69 (other cirrhosis) covers cryptogenic, macronodular, micronodular, and postnecrotic cirrhosis. Biliary cirrhosis is K74.3 (primary), K74.4 (secondary), or K74.5 (unspecified). Alcoholic cirrhosis is excluded from K74 and coded as K70.30 (without ascites) or K70.31 (with ascites), with an F10.- code for the alcohol use.
Ascites is built into the K70.3- codes but not into K74. For nonalcoholic cirrhosis with ascites, add R18.8 after the cirrhosis code. K74 carries a code-also note for viral hepatitis, so cirrhosis from chronic hepatitis C is reported with K74.60 and B18.2. Cirrhosis resulting from NASH is K74.60 or K74.69 together with K75.81.
Complications are coded separately: portal hypertension K76.6, hepatorenal syndrome K76.7, hepatopulmonary syndrome K76.81, hepatic encephalopathy K76.82 (which since FY2022 replaces K72.9- for encephalopathy without failure), spontaneous bacterial peritonitis K65.2, and hepatic failure K72.90 or K72.91 with coma. Esophageal varices secondary to cirrhosis are I85.10 or I85.11 with bleeding, and I85.1- has a code-first note for the underlying liver disease.
Alcoholic hepatic failure is K70.40 or K70.41 rather than K72. Hepatic fibrosis without cirrhosis is K74.0- (K74.00, K74.01 early, K74.02 advanced), and a liver transplant status is Z94.4 when the cirrhosis has been treated by transplant.
