How to code Cerebral Palsy

Category G80 is divided by motor pattern: G80.0 spastic quadriplegic, G80.1 spastic diplegic, G80.2 spastic hemiplegic, G80.3 athetoid (dyskinetic), G80.4 ataxic, G80.8 other, and G80.9 unspecified. The index has some non-obvious defaults: spastic cerebral palsy with no stated distribution goes to G80.1, spastic monoplegic and paraplegic also go to G80.1, and hemiplegic or diplegic cerebral palsy not described as spastic goes to G80.8. Only spastic hemiplegic reaches G80.2 and only spastic quadriplegic reaches G80.0.

Cerebral palsy is a lifelong condition and is coded at every encounter where it affects care, including adult visits. The G80 code does not carry laterality; the side of hemiplegic involvement is documented only. Hemiplegia codes from G81 are not added to G80.2, since the hemiplegia is part of the cerebral palsy.

Associated conditions are coded separately when documented: epilepsy (G40.-), intellectual disability (F70 through F79), speech and language disorders (F80.9, R47.1 dysarthria), and developmental delay (R62.50) in children without a more specific diagnosis. Mobility limitations relevant to the encounter can be captured with Z74.09.
