How to code Cardiomyopathy

I42.9 is cardiomyopathy unspecified. Specific types include I42.0- dilated cardiomyopathy (I42.00 unspecified, I42.01 familial or genetic, I42.09 other), I42.1 obstructive hypertrophic cardiomyopathy (HOCM), I42.2 other hypertrophic cardiomyopathy, I42.5 other restrictive cardiomyopathy, I42.6 alcoholic cardiomyopathy, I42.7 cardiomyopathy due to drugs or external agents, I42.81 arrhythmogenic cardiomyopathy, and I42.89 other cardiomyopathies.

Several common types are classified outside I42. Ischemic cardiomyopathy is I25.5, takotsubo (stress) cardiomyopathy is I51.81, hypertensive cardiomyopathy is reported through I11.- as hypertensive heart disease, and peripartum cardiomyopathy is O90.3. I43 (cardiomyopathy in diseases classified elsewhere) is used with the underlying disease sequenced first, such as amyloidosis (E85.-) or thyrotoxicosis.

I42.6 carries a code-also instruction for alcohol abuse or dependence (F10.-). For I42.7, code first the poisoning (T36 to T65 with fifth or sixth character 1 to 4) if applicable, or add an adverse effect code with a fifth or sixth character of 5 for a drug taken correctly, as with chemotherapy-induced cardiomyopathy.

Heart failure is not integral to cardiomyopathy. When both are documented, report the I50.- code for the heart failure as well, sequenced according to the reason for the encounter. If hypertension is also present, the I11.0 combination code is sequenced before I50.-.
