How to code Cardiomegaly
I51.7 covers cardiomegaly, cardiac dilatation, cardiac hypertrophy, and ventricular dilatation, including left ventricular hypertrophy. When the patient also has hypertension, ICD-10-CM presumes a link, so hypertensive heart disease I11.9 (or I11.0 with heart failure) is sequenced before I51.7, unless the provider documents that the enlargement is unrelated.
An enlarged heart seen only on imaging, without a clinical diagnosis from the provider, is R93.1 (abnormal findings on diagnostic imaging of heart). Enlargement due to cardiomyopathy is coded to I42.-, and congenital cardiomegaly is Q24.8.
