How to code Cannabis Use

Cannabis codes are grouped as F12.1- (abuse), F12.2- (dependence), and F12.9- (use, unspecified). F12.90 (cannabis use, unspecified, uncomplicated) is assigned when the provider documents marijuana or cannabis use without calling it abuse or dependence. Per the official guidelines, unspecified use codes should be reported only when the use is documented as relevant to the encounter and no abuse or dependence is documented.

The same use, abuse, dependence hierarchy applies as for alcohol: when two patterns are documented, code the higher one. Cannabis withdrawal is F12.13, F12.23, or F12.93 by pattern. Intoxication has uncomplicated, delirium, perceptual disturbance, and unspecified sixth characters (F12.120 to F12.129 for abuse, F12.220 to F12.229 for dependence).

Cannabis-induced anxiety disorder is F12.180, F12.280, or F12.980, and cannabis-induced psychotic disorder is coded to the 5- subcategories with delusions, hallucinations, or unspecified. Medical cannabis prescribed for a condition is not coded to F12 unless the provider documents a use disorder. A newborn affected by maternal cannabis use is P04.81 on the infant record.
