How to code Bipolar Disorder

Category F31 is built on the current episode: hypomanic (F31.0), manic without psychotic features (F31.10 to F31.13 by severity), manic with psychotic features (F31.2), depressed (F31.30 to F31.5), mixed (F31.60 to F31.64), and in remission (F31.70 to F31.78). F31.9 is bipolar disorder, unspecified, and is the correct code when the current episode is not documented.

Severity is carried in the fifth character for manic, depressed, and mixed episodes: mild, moderate, severe without psychotic features, and severe with psychotic features. For the depressed episode, F31.30 covers mild or moderate when the provider does not distinguish, F31.4 is severe without psychosis, and F31.5 is severe with psychosis.

Bipolar II disorder has its own code, F31.81, and does not use the episode structure. Remission codes require the provider to state partial or full remission and the most recent episode type; F31.70 is used when the most recent episode is unspecified.

A first manic episode with no history of depression is coded to F30 (manic episode), not F31. Cyclothymic disorder is F34.0. F39 (unspecified mood disorder) applies only when the provider cannot determine whether the disorder is bipolar or depressive.
