How to code Autism Spectrum Disorder

Autism spectrum disorder of any level is coded to F84.0 (autistic disorder). ICD-10-CM does not carry the DSM-5 severity levels (requiring support, substantial support, very substantial support) in the code, so level 1, 2, and 3 autism all map to F84.0. Document the level in the record for medical necessity but do not look for a code for it.

F84.5 (Asperger's syndrome) remains a valid code for patients with that historical diagnosis, although DSM-5 folded it into autism spectrum disorder. F84.9 (pervasive developmental disorder, unspecified) is used when the provider documents PDD-NOS or an unspecified pervasive developmental disorder. Rett's syndrome is F84.2 and childhood disintegrative disorder is F84.3.

Associated conditions are coded separately: intellectual disability (F70 through F79), language disorder (F80.9), social pragmatic communication disorder (F80.82, which is excluded from F84.0 and used only when autism criteria are not met), and ADHD (F90.-). Z13.42 is the screening code for global developmental delay and is not assigned once a diagnosis is established.
