How to code Anxiety Disorders

F41.9 (anxiety disorder, unspecified) is the default when the record says only anxiety. It is one of the most over-used codes in outpatient behavioral health, so check whether the provider named a specific disorder: F41.1 for generalized anxiety disorder, F41.0 for panic disorder, F41.3 for other mixed anxiety disorders, and F41.8 for other specified anxiety disorders.

Phobic anxiety disorders live in category F40. Agoraphobia is split by whether panic disorder is present (F40.01 with, F40.02 without, F40.00 unspecified), social phobia is F40.10 or F40.11 (generalized), and specific phobias are coded to the F40.2- subcategory by type (F40.240 claustrophobia, F40.243 fear of flying, and so on).

Anxiety caused by a medical condition is F06.4, with the physiological condition coded first. Anxiety that is a reaction to an identifiable stressor within three months is an adjustment disorder (F43.22 with anxiety, F43.23 with mixed anxiety and depressed mood), not an F41 code. Separation anxiety of childhood is F93.0.

Anxiety and depression documented together are coded separately (for example F41.1 and F33.9). There is no combination code for mixed anxiety and depression in ICD-10-CM; F41.8 applies only when the provider specifically documents mixed anxiety and depressive disorder.
