How to code ADHD

Category F90 has five billable codes: F90.0 (predominantly inattentive type), F90.1 (predominantly hyperactive type), F90.2 (combined type), F90.8 (other type), and F90.9 (unspecified type). The same codes apply to adults; there is no separate adult ADHD category. Attention deficit disorder without hyperactivity indexes to F98.8, not to F90.0, so a provider who means the inattentive presentation of ADHD should document it that way.

The DSM-5 terms inattentive presentation, hyperactive-impulsive presentation, and combined presentation map directly to F90.0, F90.1, and F90.2. When the provider documents only ADHD, assign F90.9. Documentation of stimulant therapy alone does not establish the type.

Comorbid conditions are coded in addition to F90: oppositional defiant disorder (F91.3), specific learning disorders (F81.0 reading, F81.9 unspecified), Tourette's disorder (F95.2), and autism spectrum disorder (F84.0). ADHD symptoms that are entirely explained by another disorder should not be coded as F90.
