How to code Abnormal Uterine Bleeding

Bleeding tied to the menstrual cycle is category N92 and is coded by pattern: N92.0 excessive and frequent menstruation with a regular cycle (menorrhagia), N92.1 with an irregular cycle (menometrorrhagia), N92.2 excessive bleeding at puberty, N92.3 ovulation bleeding, N92.4 excessive bleeding in the premenopausal period, N92.5 other specified irregular menstruation, N92.6 irregular menstruation unspecified. Bleeding that is not menstrual is category N93: N93.0 postcoital or contact bleeding, N93.1 pre-pubertal vaginal bleeding, N93.8 other specified, and N93.9 abnormal uterine and vaginal bleeding unspecified.

N93.9 is the correct code for a documented diagnosis of AUB or dysfunctional uterine bleeding when no pattern is stated, and it is the most common code in this group. Bleeding after menopause is always N95.0, never N93 or N92. Absent or infrequent menses are N91 codes (N91.2 amenorrhea unspecified, N91.5 oligomenorrhea unspecified), and painful menses are N94.4 through N94.6.

When a cause is established, sequence it and report the bleeding code only if the provider documents the bleeding as a separate problem: fibroids (D25.-), endometrial hyperplasia (N85.00 or N85.01), polyps (N84.-), or a coagulation disorder. Anemia from chronic menstrual loss is D50.0. Bleeding in pregnancy is chapter 15 and is never coded to N92 or N93.
