How to code Physical Therapy and Rehab Encounter

The first-listed diagnosis on an outpatient therapy claim is the condition being treated, not a Z code. A patient in PT for knee osteoarthritis is M17.11; a patient in rehab after a stroke is the I69.3x code for the residual deficit; a patient recovering from a healing fracture is the S code with 7th character D. Functional or treatment diagnoses such as M62.81 (muscle weakness), R26.89 (gait abnormality), or R53.1 (weakness) are reported as secondary codes and support the plan of care under 97161 to 97164 and 97110 to 97140.

Aftercare Z codes are first-listed only when there is no current condition to treat, meaning the patient is recovering from surgery for a condition that no longer exists. The main example is Z47.1 (aftercare following joint replacement surgery) with the Z96.6x joint status code, which is the accepted primary diagnosis for post-arthroplasty rehab. Z47.89 covers other orthopedic aftercare, Z47.81 follows an amputation, and Z48.89 is general surgical aftercare. Since FY2017, when Z47.3x and Z47.8x were added, Z47.89 rather than Z51.89 is the specific code for orthopedic aftercare.

Z51.89 (encounter for other specified aftercare) is the code that causes the most therapy denials. It is nonspecific, several Medicare contractors treat it as insufficient on its own for therapy medical necessity, and it should never be the only diagnosis on the claim. When it is used, it belongs behind the condition or the more specific aftercare code. Z09 is not a therapy code at all; it describes a follow-up exam after completed treatment, and a patient still in therapy is not finished with treatment.

Z74.09 (other reduced mobility) and Z91.81 (history of falling) are useful secondary codes for balance and mobility programs, and the referral diagnosis from the ordering provider should be carried onto the therapy claim so the plan of care and the certification match.
