How to code Depression Screening
Z13.31 (encounter for screening for depression) is the diagnosis for a standardized depression screen such as the PHQ-2 or PHQ-9 administered to a patient with no current diagnosis of depression. It is billed with 96127 (brief emotional or behavioral assessment) for most payers and with G0444 for Medicare's annual depression screening, which must be done in a primary care setting with staff-assisted follow-up available. Z13.31 is usually a secondary code under Z00.00 or Z00.01 at a wellness visit, or under the problem that brought the patient in; it can be first-listed only when the screen is the sole reason for the encounter.
Z13.32 (screening for maternal depression) is the code for postpartum depression screening, including the screen performed on the mother during the infant's well visit. When the mother is screened at the baby's visit, the service is reported on the infant's claim with 96161 (caregiver-focused health risk assessment) and Z13.32, and the infant's own Z00.12x code stays first-listed.
A screening code is wrong once the patient has depression. A PHQ-9 given to monitor a patient already treated for major depression is part of managing F32.x or F33.x and does not get Z13.31, even though 96127 can still be billed for the instrument. A positive screen at the visit is recorded with the screening code first and the finding second: F32.A (depression, unspecified) if the provider documents depression, or R45.851 if suicidal ideation is identified. Z13.39 covers other mental health screens such as the GAD-7 for anxiety, AUDIT for alcohol, or the substance use screen billed with G0442 or 99408.
The pediatric developmental screens live in Z13.4x: Z13.41 for autism screening (M-CHAT), Z13.42 for milestone screening (ASQ), Z13.40 and Z13.49 for other developmental delays, all billed with 96110. Z13.30 is the unspecified mental health screen and should be avoided when the instrument is documented.
