Well-Woman Exam ICD-10-CM codes

Z01.419 (gynecological examination without abnormal findings) is the code for the routine annual well-woman visit. It already includes the pelvic exam, the clinical breast exam, and a pap smear taken as part of the routine exam, so do not add Z12.4 to a Z01.41x visit; Z01.41- carries an Excludes1 note against Z12.4 for exactly that reason. When the exam finds something new, report Z01.411 with a secondary code for the finding, for example R87.610 for ASC-US on the smear if the result is back at the time of coding.

Z01.4x is first-listed only. If the patient is seen for a gynecologic complaint rather than a routine exam, code the complaint (abnormal bleeding, pelvic pain, vaginitis) and drop the Z code. Z01.42 is reserved for the repeat smear ordered to confirm a normal result that followed an earlier abnormal smear; it is not for the routine annual pap and not for a diagnostic colposcopy visit.

Commercial payers pair Z01.419 with the preventive visit codes 99385 to 99397. Medicare does not pay for a preventive visit code, but it covers the pelvic and clinical breast exam under G0101 and the pap collection under Q0091, billed with Z01.419, Z01.411, or Z12.4 (every 24 months at low risk, annually for high-risk patients with a Z72.5x code). HPV co-testing gets Z11.51 as an additional code.

Services performed at the same visit take their own codes: Z30.09 or a Z30.4x code for contraceptive counseling or surveillance, Z12.72 and Z12.73 for vaginal or ovarian cancer screening, and Z78.0 for the asymptomatic menopausal state. A patient with a history of hysterectomy (Z90.710) who is seen after treatment for gynecologic cancer belongs under Z08 rather than Z01.4x.
