How to code Lab Work Encounter
There is no single blood work code, and the most frequent claim error on lab encounters is reaching for one. The diagnosis on a lab claim is the reason the test was ordered. A basic metabolic panel to monitor a patient's lisinopril is coded to I10; an A1c for a diabetic is E11.9; a TSH for hypothyroidism is E03.9. These are diagnostic tests and the condition, not a Z code, supports medical necessity under the payer's lab policy.
Z codes apply when the patient has no condition or symptom that justifies the test. Routine labs drawn as part of the annual physical are covered by Z00.00 or Z00.01 and need no separate code. Labs drawn for screening in an asymptomatic patient use the matching Z13 code: Z13.1 for glucose or A1c, Z13.220 for a lipid panel (Z13.6 for Medicare), Z13.0 for a CBC, Z13.21 for a nutritional screen, Z13.228 for other metabolic screens, and Z13.29 for an endocrine screen such as TSH. Z13.9 (screening, unspecified) is accepted by few payers and should be replaced with a specific code whenever the order names the test.
Pre-operative labs are Z01.812 (encounter for preprocedural laboratory examination), first-listed, followed by the condition requiring surgery and then the comorbidities. Blood typing is Z01.83, antibody titers for immunity or employment are Z01.84, and Z01.89 covers other special examinations. Do not report Z01.812 for routine labs unrelated to a planned procedure.
When the visit is for a drug level or a safety lab required by a medication, Z51.81 (encounter for therapeutic drug level monitoring) is first-listed with the Z79 code for the drug and the condition after it: Z51.81, Z79.01, I48.91 for an INR check, or Z51.81, Z79.899, E78.5 for a statin liver panel. Vitamin D testing (82306) is a frequent denial under Z13.21 because most carriers require a diagnosis such as E55.9 or M81.0, so confirm the payer's LCD before billing it as a screen.
