How to code Pap Smear and Cervical Cancer Screening
Which code goes first depends on whether the pap was the whole visit or part of a routine gynecological exam. A visit only for the pap smear, or a pap collected during an unrelated problem visit, is Z12.4 (encounter for screening for malignant neoplasm of cervix). A pap collected during the annual well-woman exam is included in Z01.419 or Z01.411, and Z12.4 is not added, because subcategory Z01.41 has an Excludes1 note for Z12.4. HPV co-testing is reported with Z11.51 as an additional code in either situation.
Z01.42 is a narrow code: the repeat smear ordered to confirm a normal result after an earlier abnormal one. Once a smear is abnormal, the follow-up colposcopy or repeat cytology is coded to the abnormal finding, R87.610 for ASC-US, R87.612 for LSIL, R87.810 for a positive high-risk HPV test, not to a screening code. Reporting Z12.4 on a diagnostic pap after an abnormal result is a frequent denial.
On the lab side, the screening pap is billed with 88141 to 88175 depending on method, and Medicare pays the collection under Q0091 with the pelvic exam under G0101 every 24 months for low-risk patients. High-risk patients are covered annually, and Medicare expects one of the Z72.5x high-risk sexual behavior codes as the second diagnosis to justify the yearly interval. A patient who has had a total hysterectomy (Z90.710) for a benign reason no longer needs cervical screening; if the hysterectomy was for cancer, ongoing surveillance is Z08 with Z85.41.
Z86.001 (personal history of in-situ neoplasm of cervix) supports more frequent screening after treated CIN. Screening for other gynecologic cancers uses Z12.72 (vagina) and Z12.79 (other genitourinary organs); Z12.4 is strictly cervical.
