Pre-Operative Clearance ICD-10-CM codes

For a visit whose sole purpose is a pre-operative evaluation, the official guidelines (section IV.M) require a code from subcategory Z01.81 as the first-listed diagnosis. Z01.818 is the general pre-op history and physical, Z01.810 is a cardiovascular clearance (EKG, cardiology consult), Z01.811 is a respiratory clearance (chest x-ray, pulmonary function testing), and Z01.812 covers pre-op labs. Report the condition that is the reason for the surgery as an additional diagnosis, then any findings from the pre-op evaluation, then relevant chronic conditions.

Documentation must name the planned procedure, the surgeon who requested the clearance, and the condition being operated on. A note that says only pre-op clearance with no surgical diagnosis is the leading cause of denials on these visits. The clearance visit itself is reported with an office or consultation E/M code; the surgeon's own pre-op H&P within the global period is not separately billable.

Medical necessity for pre-op testing is judged by the payer against the patient's comorbidities, not against the Z code alone. An EKG with Z01.810 on a healthy 30-year-old will be denied by most carriers; the same EKG with Z01.810, the surgical diagnosis, and I10 or I48.91 is usually covered. Pair the Z01.81x code with the comorbidity that makes the test reasonable.

If the procedure is later cancelled, the cancellation reason is coded with category Z53: Z53.01 when the patient smoked, Z53.09 for another contraindication found at clearance, Z53.20 or Z53.29 for the patient's decision, and Z53.9 when the reason is not documented. Z01.81x is also not for the routine post-op visit; that is aftercare under Z47 or Z48.
