Transplant Status ICD-10-CM codes

ICD-10-CM codes for transplanted organ and tissue status (Z94), aftercare following transplant (Z48.2x), and the immunosuppressant use codes that accompany them, with the rule for when a complication code replaces the status code.

31 codes, 26 billable. FY2027, valid from October 1, 2026. In chapter 21: Factors Influencing Health Status and Contact with Health Services.

How to code Transplant Status

Z94 is the status category for a functioning transplant: Z94.0 kidney, Z94.1 heart, Z94.2 lung, Z94.3 heart and lungs, Z94.4 liver, Z94.5 skin, Z94.6 bone, Z94.7 cornea, Z94.81 bone marrow, Z94.82 intestine, Z94.83 pancreas, Z94.84 stem cells. It is a secondary code reported on every encounter where the transplant is relevant, which for a solid organ recipient is nearly every visit, because the transplant and its immunosuppression change infection risk, drug choices, and risk adjustment.

The guidelines (section I.C.21.c.3) are explicit that a status code is not used with a complication code for the same organ: a patient with kidney transplant rejection is T86.11, not T86.11 plus Z94.0, and a patient with an unspecified complication is T86.10. For kidney recipients, CKD after transplant is not itself a complication, since a transplanted kidney may never restore full function; report the N18 stage with Z94.0 unless the provider documents rejection or failure.

Routine post-transplant visits during the recovery phase are aftercare, first-listed under Z48.2x: Z48.21 heart, Z48.22 kidney, Z48.23 liver, Z48.24 lung, Z48.280 heart-lung, Z48.290 bone marrow, Z48.298 other. Z94 follows as the status. Once recovery is complete and the visits are long-term surveillance, the status code with the conditions being managed is enough, and Z48.2x is dropped.

Immunosuppression is coded separately with the Z79.6x series: Z79.621 for calcineurin inhibitors (tacrolimus, cyclosporine), Z79.623 for mTOR inhibitors, Z79.620 for biologics, Z79.60 when the agent is not specified. These support therapeutic drug monitoring under Z51.81 and explain the increased infection surveillance. Bone marrow and stem cell recipients with graft-versus-host disease are coded D89.81x with the T86.0x code first, again without Z94.81.
