How to code Tobacco Use and Smoking

Three codes cover the current or past smoker, and the choice depends on the words in the record. A patient documented as a smoker, cigarette smoker, or nicotine dependent is F17.2x, because the Alphabetic Index sends the term smoker to nicotine dependence: F17.210 for cigarettes, F17.220 for chewing tobacco, F17.290 for other products including vaping, and F17.200 when the product is not named. Z72.0 (tobacco use) applies only when the provider specifically documents tobacco use without dependence, which is uncommon. Z87.891 (personal history of nicotine dependence) is the former smoker who has quit.

The sixth character of F17.2x adds the state: 0 uncomplicated, 1 in remission (the provider must document remission; a patient who quit last month is still dependent unless stated otherwise), 3 with withdrawal, 8 with other nicotine-induced disorders, and 9 with unspecified nicotine-induced disorders. Coders may not choose remission on their own, and a patient on nicotine replacement is still F17.2x, not Z79.

Tobacco status is required as an additional code by dozens of chapters. COPD, lung cancer, coronary disease, peptic ulcer, and pregnancy chapters all carry use additional code notes for F17.-, Z72.0, Z87.891, or exposure codes. Z77.22 (exposure to environmental tobacco smoke) covers secondhand smoke at home, Z57.31 is occupational exposure, and Z81.2 is family history of tobacco dependence. Report the tobacco code on every claim where the underlying condition instructs it; it is also a risk adjustment element.

Cessation counseling is billed with 99406 (3 to 10 minutes) or 99407 (more than 10 minutes), paired with F17.2x as the diagnosis. Z71.6 (tobacco abuse counseling) can be added but is not required and is not accepted alone by most payers. For Medicare, the counseling benefit covers up to 8 sessions in 12 months and the claim must carry the F17 or Z72.0 code that establishes current use.
