How to code Aftercare Following Surgery

Aftercare codes (section I.C.21.c.7) cover visits after the initial treatment of a condition when the patient needs continued care during recovery. Category Z48 handles general post-surgical care: Z48.01 for a surgical dressing change, Z48.02 for suture or staple removal, Z48.03 for drain care, Z48.1 for planned delayed wound closure, Z48.3 for aftercare following surgery for a neoplasm, and the Z48.81x codes for aftercare by body system (Z48.812 after cardiac surgery, Z48.815 after digestive surgery, Z48.816 after genitourinary surgery). Z48.89 is the catch-all for other specified surgical aftercare. Z48.2x is aftercare following an organ transplant.

Orthopedic aftercare is category Z47. Z47.1 is the aftercare visit after a joint replacement and requires an additional Z96.6x code to identify the joint (Z96.651 for a right knee, Z96.641 for a right hip). Z47.2 is the planned removal of an internal fixation device after the fracture has healed, Z47.3x covers aftercare after explantation of a joint prosthesis, Z47.81 follows a surgical amputation, Z47.82 follows scoliosis surgery, and Z47.89 is other orthopedic aftercare.

The fracture rule is the one coders get wrong most often: aftercare Z codes are never used for injuries. A patient returning for a cast change or x-ray during routine healing of a wrist fracture is coded S52.501D (the fracture code with 7th character D, subsequent encounter for routine healing), not Z47.89 and not Z09. Use 7th character G for delayed healing, K for nonunion, P for malunion, and S for sequela. The 7th character A is reserved for active treatment: the ED visit, the surgery, or the first evaluation by a new physician. Aftercare Z codes apply only to conditions without a 7th character, which is why post-surgical aftercare for a joint replacement done for arthritis uses Z47.1 but the same rehab after a hip fracture repair uses S72.001D.

Aftercare Z codes are also not used when treatment is directed at a current, acute condition. A wound infection at the post-op visit is T81.41XA, a dehiscence is T81.31XA, and the aftercare code is dropped. Within the surgeon's global period the aftercare visit is reported with 99024 and is not separately paid; Z48 and Z47 codes still belong on the encounter for documentation and for the non-surgeon providers (wound care, PT, home health) who are paid for their aftercare services.
