How to code Social Determinants of Health (SDOH)

The SDOH codes are secondary codes reported whenever the social factor is documented and relevant to care. Section I.C.21.c.17 sets a permissive documentation standard: the code may be assigned from the documentation of any clinician involved in the patient's care, including social workers, community health workers, case managers, and nurses, and from patient self-reported screening data as long as a clinician signs off and incorporates it into the record. The provider does not need to restate the finding, which makes these codes different from most diagnoses.

The housing and economic codes in Z59 are the most used. Homelessness is Z59.01 (sheltered), Z59.02 (unsheltered), or Z59.00 (unspecified). Housing instability in a housed patient is Z59.811 (risk of homelessness), Z59.812 (homeless in the past 12 months), or Z59.819. Inadequate housing is Z59.1x, food insecurity is Z59.41 (Z59.48 for other lack of adequate food), transportation insecurity is Z59.82, financial insecurity is Z59.86x with Z59.861 for difficulty paying utilities, and material hardship is Z59.87. Z59.71 covers insufficient health insurance and Z59.6 low income. Z59.5 (extreme poverty) requires that specific documentation.

The other categories fill out the picture: Z55 education and literacy (Z55.6 for problems with health literacy), Z56 employment (Z56.0 unemployment, Z56.82 military deployment status), Z57 occupational exposures, Z58 physical environment (Z58.6 inadequate drinking water), Z60 social environment (Z60.2 living alone, Z60.4 social exclusion), Z62 upbringing including the personal history of childhood abuse codes Z62.81x, Z63 family circumstances (Z63.0 partner relationship problems, Z63.4 death of a family member, Z63.6 dependent relative needing care), Z64 unwanted pregnancy and multiparity, and Z65 legal and psychosocial circumstances such as incarceration (Z65.1) and release from prison (Z65.2).

These codes support the SDOH risk assessment (G0136 for Medicare, 96160 or 96161 for other payers), community health integration services (G0019, G0022), and principal illness navigation, and they feed risk adjustment and quality programs even though they carry no HCC weight. Payers do not deny claims for the presence of an SDOH code, but several state Medicaid programs and Medicare quality measures now require them, and a screen documented in the record without the code is a missed reporting opportunity. Z59.x codes reported as first-listed are rare and generally limited to a visit solely for social services.
