How to code Pregnancy Supervision (Normal and High Risk)

Routine prenatal visits in an uncomplicated pregnancy are coded to category Z34, first-listed, and never together with a chapter 15 O code. Z34.0x is the first pregnancy, Z34.8x is a subsequent normal pregnancy, and Z34.9x is used when the provider does not document whether it is the first. The fifth character is the trimester: 1 for first, 2 for second, 3 for third, 0 for unspecified. Trimester is counted from the first day of the last menstrual period (first trimester less than 14 weeks 0 days, second 14 to less than 28 weeks, third 28 weeks to delivery) and should be documented at every visit, so the unspecified trimester codes should be rare.

When the provider documents the pregnancy as high risk, routine prenatal visits use category O09 as the first-listed diagnosis instead of Z34 (section I.C.15.b.2). O09 is subdivided by the reason: O09.0x history of infertility, O09.1x history of ectopic pregnancy, O09.21x history of preterm labor, O09.29x other poor obstetric history, O09.3x insufficient prenatal care, O09.4x grand multiparity, O09.51x and O09.52x elderly primigravida or multigravida (35 or older at the expected delivery date), O09.61x and O09.62x young primigravida or multigravida (under 16), O09.81x pregnancy from assisted reproduction, O09.89x other high-risk pregnancy, and O09.9x unspecified. Other chapter 15 codes may follow O09 when a specific complication is also present.

Once a complication is diagnosed, the visits for that complication are coded to the complication, not to Z34. A patient with gestational diabetes is seen under O24.4x, a patient with pregnancy-induced hypertension under O13.x, and Z34 is dropped for those encounters. The combination of Z34 with any O code on the same claim is an edit failure. Z34 also does not apply to a pregnant patient seen for an unrelated problem: code the problem with O99.89 or the relevant O code, and Z33.1 when the pregnancy is incidental to the visit.

Global obstetric packages (59400, 59510) bundle the routine visits, so the Z34 or O09 code is reported on the delivery claim with the O80 or delivery outcome codes and Z37.x, and on any separately billed antepartum visits (59425, 59426). Z3A weeks of gestation codes go on the maternal record when documented, and Z39.2 covers the routine postpartum check after the global period ends.
