How to code Palliative Care and DNR Status

Z51.5 (encounter for palliative care) is reported whenever the record documents palliative care, comfort care, hospice care, or end-of-life care. It is normally a secondary code, with the terminal or life-limiting condition sequenced first: C34.90 with Z51.5 for a lung cancer patient transitioned to comfort measures, or I50.9 with Z51.5 for end-stage heart failure. In the inpatient setting Z51.5 is excluded from the principal diagnosis by most payers even though the guidelines do not forbid it, and its presence removes the admission from several CMS mortality measures, so it should be captured every time the documentation supports it.

Z66 (do not resuscitate) is a status code assigned when a DNR or no-code order is documented at any point during the encounter. It is secondary only, it is not the same as comfort care (a patient can be DNR and still receive full treatment), and it is not the same as a POLST form or an advance directive that has not been activated into an order. Z66 affects risk adjustment for inpatient mortality measures and belongs on the claim whenever the order exists, including when the order is written on day one and the patient later dies.

Advance care planning and goals-of-care discussions billed with 99497 and 99498 have no dedicated diagnosis; report the condition that prompted the discussion, or Z71.89 (other specified counseling) when the conversation is not tied to one disease. Hospice patients seen by an outside physician for something unrelated to the terminal illness use modifier GV or GW with the unrelated diagnosis.

The Z74 codes describe care dependency and are often relevant to palliative and hospice patients: Z74.01 bed confinement, Z74.09 other reduced mobility, Z74.1 need for help with personal care, Z74.2 need for help at home with no one available, Z74.3 need for continuous supervision. Z99.81 (dependence on supplemental oxygen) and Z91.81 (history of falling) round out the functional picture and support home health and DME certification.
