How to code Lung Nodule
R91.1 is solitary pulmonary nodule, including a coin lesion. Multiple lung nodules and other nonspecific abnormal lung findings on imaging are R91.8.
A nodule found on a lung cancer screening CT is coded with Z12.2 for the screening and R91.1 or R91.8 for the finding. Add Z87.891 for a history of smoking when documented.
Once the nodule is diagnosed, the R91 code is replaced by the diagnosis, such as C34.90 for lung cancer or D14.30 for a benign lung neoplasm.
