How to code Nausea and Vomiting

R11.2 (nausea with vomiting, unspecified) is the most common code. R11.0 is nausea alone. Vomiting without nausea has its own subcategory: R11.10 vomiting unspecified, R11.11 vomiting without nausea, R11.12 projectile vomiting, R11.13 vomiting of fecal matter, R11.14 bilious vomiting, R11.15 cyclical vomiting syndrome unrelated to migraine, and R11.16 cannabis hyperemesis syndrome. When cyclical vomiting is part of migraine, use G43.A- (G43.A0 not intractable) instead of R11.15. With R11.16, add the F12 code for cannabis use (F12.90 when unspecified).

Several forms of vomiting are excluded from R11 and coded elsewhere: vomiting in pregnancy is O21.- (O21.9 unspecified), hematemesis is K92.0, vomiting following gastrointestinal surgery is K91.0, psychogenic vomiting is F45.8, and vomiting of the newborn is P92.0- (P92.09 other). Dehydration resulting from vomiting is E86.0 and is coded in addition.

When gastroenteritis or another cause is confirmed (A09 infectious gastroenteritis, K52.9 noninfective gastroenteritis), the R11 code is not reported. A symptom documented as integral to a confirmed diagnosis, such as nausea with a documented bowel obstruction, is likewise not coded separately.
