How to code Chest Pain

R07.9 (chest pain, unspecified) is the default when the provider documents chest pain without further description. More specific options are R07.1 chest pain on breathing (painful respiration), R07.2 precordial pain, R07.81 pleurodynia, R07.82 intercostal pain, and R07.89 other chest pain, which covers chest wall pain, musculoskeletal chest pain, and atypical chest pain. R07.0 is pain in the throat.

A symptom code is not reported when a definitive diagnosis that explains it has been established. Chest pain attributed to angina is I20.- (I20.9 unspecified, I20.0 unstable), to a myocardial infarction is I21.-, and to reflux is K21.9; in each case the R07 code is dropped. In the outpatient setting a diagnosis qualified as probable, suspected, or rule out is not coded, so the symptom code R07.9 is reported instead. For inpatient admissions the uncertain diagnosis is coded as if it existed at the time of discharge.

Chest pain that is documented as integral to a confirmed condition (for example pleuritic pain with pneumonia) is not coded separately. Chest pain that persists after workup with no cause found stays R07.9 or the more specific R07 code.
