How to code Dizziness and Syncope

R42 is a single code for dizziness, giddiness, and lightheadedness, and for vertigo when the provider does not identify a vestibular cause. R55 is a single code for syncope and collapse, which includes fainting, blackout, vasovagal attack, and near-syncope or presyncope. Neither code has additional characters.

Once the cause is documented, code it and drop the symptom. True vertigo with a vestibular diagnosis goes to H81: benign paroxysmal positional vertigo is H81.1- with laterality (H81.10 unspecified ear, H81.11 right, H81.13 bilateral). Syncope from orthostatic hypotension is I95.1, from bradycardia R00.1 or the specific arrhythmia, from carotid sinus hypersensitivity G90.01, from postural orthostatic tachycardia syndrome G90.A, from coughing R05.4, and from heat T67.1XXA. Hypotension without a stated type is I95.9.

A transient alteration of awareness without a loss of consciousness is R40.4. Syncope with a witnessed seizure is coded to the seizure diagnosis. When the workup is complete and no cause is found, R55 or R42 remains the final code. Add external cause and place codes when a fall or injury resulted from the syncope, and code the injury first when it is the reason for the encounter.
