How to code Long-Term Anticoagulant Use

Z79.01 (long-term current use of anticoagulants) is reported as a secondary code for any patient on warfarin, apixaban, rivaroxaban, dabigatran, edoxaban, or long-term enoxaparin. The reason for anticoagulation is coded first and must appear on the claim: I48.x for atrial fibrillation, I82.x or I26.x for a current DVT or PE, Z86.718 or Z86.711 for a history of DVT or PE that is no longer active but keeps the patient on prophylaxis, Z95.2 or Z95.4 for a prosthetic heart valve. Z79.01 alone, without the indication, is the most common reason an anticoagulation claim is returned.

Z79.02 covers antithrombotic and antiplatelet agents such as clopidogrel, prasugrel, and ticagrelor, and Z79.82 covers daily aspirin. A patient on dual antiplatelet therapy after a stent is Z95.5 with Z79.02 and Z79.82. The guidelines limit Z79 to extended therapy, so the short post-operative course of heparin or enoxaparin for VTE prophylaxis does not get Z79.01.

The INR check is its own encounter type. When the visit is solely for a drug level, Z51.81 (encounter for therapeutic drug level monitoring) is first-listed, then Z79.01, then the indication: Z51.81, Z79.01, I48.91. This sequence supports 85610 (prothrombin time) and the anticoagulation management codes 93792 and 93793. Bleeding on therapy is D68.32 (hemorrhagic disorder due to extrinsic circulating anticoagulants) with T45.515A for the adverse effect and the site of bleeding, and Z79.01 is still added to show the patient remains on the drug.

Underdosing or missed doses that led to a clot are captured with Z91.128 (intentional underdosing) or Z91.148 (other noncompliance) along with the T45.516A underdosing code and the resulting condition. Z86.72 records a history of thrombophlebitis. Z79.01 should be reviewed at every visit and dropped once the provider documents that anticoagulation has been stopped.
