How to code Counseling Encounter
Category Z71 is for visits where counseling is the service, not an incidental part of managing a disease. Section I.C.21.c.10 says counseling codes are not used when the counseling is integral to standard treatment, so the diabetic who receives diet advice at the diabetes visit gets E11.9 alone, while a visit dedicated to nutrition counseling gets Z71.3. The main codes: Z71.3 dietary counseling, Z71.41 alcohol counseling for the patient, Z71.42 for a family member, Z71.51 and Z71.52 drug abuse counseling, Z71.6 tobacco counseling, Z71.7 HIV counseling, Z71.82 exercise counseling, Z71.83 nonprocreative genetic counseling, Z71.84 travel health counseling, Z71.85 immunization safety counseling, Z71.87 pediatric-to-adult transition counseling, Z71.88 counseling for socioeconomic factors, Z71.89 other specified counseling, and Z71.9 unspecified.
Sequencing depends on whether a condition is present. Z71.3 is first-listed for medical nutrition therapy (97802 to 97804) in a patient being counseled for prevention; when the patient has obesity, hyperlipidemia, or diabetes, the condition is first-listed and Z71.3 follows. Medicare's obesity counseling (G0447) needs E66.x with the Z68 code, and its alcohol misuse counseling (G0443) follows a positive screen (G0442) with the F10.x or Z71.41 code. Tobacco counseling (99406, 99407) is paired with F17.2x, and Z71.6 is optional.
The three codes at the top of the category describe unusual encounters. Z71.0 is a visit where someone consults on behalf of another person who is not present. Z71.1 is a patient with a feared complaint in whom no diagnosis is made, the worried-well visit. Z71.2 is a visit solely to explain test results already obtained. All three are first-listed and none supports a high-level E/M on its own.
Genetic counseling before or after testing (96040) is Z71.83 when it is not for reproductive planning; procreative genetic counseling is Z31.5. Z71.84 supports the travel clinic visit and is paired with Z23 when vaccines are given. Z71.89 is the fallback for advance care planning discussions and other counseling that has no specific code, and should be replaced by a specific Z71 code whenever one fits.
