How to code Contraception Management and Sterilization

Category Z30 is organized by what happened at the visit. Counseling only is Z30.09 (Z30.02 for natural family planning). The first prescription of a method is Z30.01x: Z30.011 pills, Z30.012 emergency contraception, Z30.013 injectable, Z30.014 IUD, Z30.015 vaginal ring, Z30.016 patch, Z30.017 implant, Z30.018 other. Ongoing management is Z30.4x: Z30.41 pills, Z30.42 injectable (the Depo-Provera visit, with 96372 and J1050), Z30.44 ring, Z30.45 patch, Z30.46 implant. These codes are first-listed when contraception is the reason for the visit and secondary when the counseling happens during a well-woman exam or another problem visit.

IUD encounters have their own codes under Z30.43: Z30.430 for insertion (58300 plus the device J code), Z30.431 for a routine check, Z30.432 for removal (58301), and Z30.433 for removal and reinsertion at the same visit. Z30.014 is for the visit where the IUD is prescribed or ordered before insertion; on the insertion day use Z30.430. Z97.5 (presence of IUD) is a status code for an IUD noted incidentally at an unrelated encounter. Z79.3 (long-term use of hormonal contraceptives) is added when the patient's hormonal method is relevant to another problem being managed.

Elective sterilization is Z30.2 (encounter for sterilization) as the first-listed diagnosis for a tubal ligation (58600, 58670, 58671), salpingectomy for sterilization, or vasectomy (55250). Medicaid and many commercial plans require the signed sterilization consent form on file, with the 30-day waiting period met, for the claim to pay. After the procedure the patient's status is Z98.51 (tubal ligation status) or Z98.52 (vasectomy status), reported as secondary codes when relevant. Sterilization reversal is Z31.0 with Z31.42 for the aftercare.

Z30.8 and Z30.9 (other and unspecified contraceptive management) should be replaced with the specific code whenever the method is documented. Contraception provided for a medical reason such as menorrhagia is coded to the condition (N92.0) with the Z30 code second, and a patient seen for a complication of the method, for example a displaced IUD, is coded to the complication (T83.32XA) rather than Z30.
