How to code Shoulder Pain

Shoulder pain is M25.51-: M25.511 pain in right shoulder, M25.512 pain in left shoulder, and M25.519 pain in unspecified shoulder. Pain in both shoulders takes both side codes. Add G89.29 when the provider documents the pain as chronic.

M25.51- is not used once a cause is documented. Frozen shoulder (adhesive capsulitis) is M75.01 right or M75.02 left, a nontraumatic rotator cuff tear is coded from M75.1-, such as M75.101 or M75.102, impingement syndrome is M75.4-, such as M75.41, shoulder bursitis is M75.5-, and primary shoulder osteoarthritis is M19.011 or M19.012. Shoulder stiffness without a diagnosis is M25.611 or M25.612.

Pain in the upper arm rather than the joint is M79.62-, such as M79.621. Pain that radiates to the shoulder from the neck is often documented as cervicalgia, M54.2; code what the provider states rather than assuming a site.
