How to code Muscle Spasm

Muscle spasm is M62.83-: M62.830 muscle spasm of back, M62.831 muscle spasm of calf, and M62.838 other muscle spasm, which covers sites such as the neck, shoulder, or thigh. M62.83 itself is not billable.

R25.2 (cramp and spasm) is a symptom code used for cramps, such as nocturnal leg cramps, when the provider does not describe a muscle spasm. Do not report R25.2 and M62.83- for the same complaint.

Back spasm often comes with back pain. When both are documented, M62.830 can be reported with the pain code for the region, such as M54.50 for low back pain, M54.6 for thoracic pain, or M54.2 for neck pain. Muscle pain without spasm is myalgia, M79.10 or M79.18, and spasmodic torticollis (cervical dystonia) is G24.3.
