How to code ENT

Sinusitis coding separates acute (J01.90, or the site-specific J01 codes) from chronic (J32.9 and the J32 site codes); chronic sinusitis paired with nasal polyps (J33.9) or deviated septum (J34.2) is the diagnostic cluster that supports endoscopic sinus surgery authorization, so code every documented structural finding. Allergic rhinitis should be coded to its trigger when known: J30.1 for pollen rather than J30.9.

Cerumen impaction requires laterality for removal claims: H61.21 right, H61.23 bilateral, with H61.20 reserved for unspecified documentation. The same laterality discipline applies across ear disease: otitis media (H66.90 unspecified, H66.001 acute suppurative right ear), otitis externa (H60.90), otalgia (H92.09), and tinnitus (H93.19). Hearing loss coding distinguishes bilateral sensorineural loss (H90.3) from unspecified (H91.90), which matters for audiology and hearing aid coverage.

Tonsil and adenoid surgery is supported by J35.1 (hypertrophy of tonsils), J35.3 (hypertrophy with adenoids), and recurrent infection history; acute episodes are J03.90 for tonsillitis and J02.0 when strep is confirmed. Vertigo workups use H81.10 (benign paroxysmal vertigo) once diagnosed and R42 (dizziness) while undifferentiated. Obstructive sleep apnea (G47.33) and snoring (R06.83) anchor the sleep surgery population; the OSA code requires a sleep study result in the record.
