How to code Pediatrics

Well-child visits are the highest-volume pediatric service: Z00.129 for a routine exam without abnormal findings, Z00.121 when a new or worsening finding is documented (code the finding too). Z23 is reported once per visit for immunization administration regardless of the number of vaccines given, and it can accompany the well-child code.

Acute infections fill the rest of the schedule. Distinguish unspecified URI (J06.9) from the specific sites: acute otitis media (H66.90, or H66.001 for suppurative with laterality), streptococcal pharyngitis (J02.0) versus unspecified sore throat (J02.9), croup (J05.0), and bronchiolitis (J21.9, or J21.0 when RSV is confirmed). Confirmed influenza with respiratory manifestations is J11.1; B34.9 covers the nonspecific viral illness when no site or organism is documented.

Developmental and behavioral coding grows with the panel age: ADHD by presentation (F90.0 inattentive, F90.1 hyperactive, F90.2 combined), autism spectrum disorder (F84.0), speech and language delay (F80.9), failure to thrive (R62.51), and nocturnal enuresis (N39.44). For pediatric obesity, pair E66.9 with a pediatric BMI percentile code such as Z68.54 (95th percentile and above), never the adult Z68.3x and Z68.4x series.
