How to code Podiatry

Routine foot care (nail debridement, callus paring) is only covered when a qualifying systemic condition puts the patient at risk, so claims pair the foot diagnosis (B35.1 onychomycosis, L60.2 onychogryphosis, L84 corns and callosities, L60.3 nail dystrophy) with the systemic condition and its class findings: diabetes with neuropathy (E11.42 or E11.40) or peripheral angiopathy (E11.51, E11.52), or peripheral vascular disease (I73.9). Missing the systemic code is the top routine-foot-care denial.

Diabetic foot ulcers require two codes in sequence: the diabetes combination code E11.621 (Type 2 with foot ulcer) first, then an L97 code identifying site, laterality, and severity, such as L97.419 (right heel and midfoot) or L97.529 (other part of left foot). The L97 severity character (breakdown, fat layer exposed, muscle, bone) must match the wound documentation. Gangrene escalates to the E11.52 combination plus I96 when not diabetic in origin.

Heel pain is dominated by plantar fasciitis (M67.A01 right, M67.A02 left, new in FY2027; it was M72.2 before), Achilles tendinitis (M76.61, M76.62), and calcaneal spur (M77.31, M77.32). Morton's neuroma is coded as a plantar nerve lesion, G57.61 or G57.62 by side. Forefoot deformity codes carry laterality: hallux valgus M20.11 and M20.12, bunionette M21.611 and M21.612. For nonspecific complaints, the foot and toe pain codes M79.671 to M79.675 are side- and site-specific; avoid the unspecified versions on surgical claims.
