Orthopedics ICD-10-CM codes
The diagnosis codes orthopedic practices report most often: osteoarthritis by site and laterality, spine and joint pain, rotator cuff and tendon disorders, and common fractures and sprains.
58 codes, 58 billable. FY2027, valid from October 1, 2026.

How to code Orthopedics
Laterality is the top denial driver in orthopedic coding. Nearly every joint code requires right, left, or bilateral: knee OA is M17.11 (right), M17.12 (left), or M17.0 (bilateral primary), and unspecified-side codes like M17.9 are routinely rejected on surgical claims. The same applies to joint pain (M25.511 right shoulder, M25.561 right knee), rotator cuff tears (M75.101, M75.102), and lateral epicondylitis (M77.11, M77.12).
Injury codes from chapter 19 require a 7th character on every claim: A for the initial active-treatment encounter, D for routine healing follow-up, and S for sequela. A distal radius fracture is S52.501A at the first cast visit but S52.501D at cast checks; billing A for the entire episode is a common audit finding. Sprains follow the same pattern (S93.401A right ankle, S83.511A ACL).
Spine coding pairs the pain code with the structural diagnosis when both are documented: M54.50 or M54.51 (vertebrogenic) for low back pain, M54.16 for lumbar radiculopathy, M48.061 and M48.062 for lumbar stenosis without and with neurogenic claudication, and M51.26 for lumbar disc displacement. The stenosis split matters because M48.062 supports decompression surgery authorization.
Postoperative encounters use Z47.1 (aftercare following joint replacement) with the joint status code: Z96.641 and Z96.642 for right and left hip prostheses, Z96.651 and Z96.652 for knees. For fragility fracture patients, M81.0 covers osteoporosis without current fracture and Z87.310 the history of a healed osteoporotic fracture.
