How to code Elevated Troponin

The index sends elevated troponin to R79.89 (other specified abnormal findings of blood chemistry). Use it only when the provider documents the lab value without a cause or a diagnosis.

When the provider documents non-ischemic myocardial injury, use I5A instead, and code the underlying cause, such as sepsis or kidney disease, as well. Demand ischemia without infarction is I24.89.

When troponin elevation is diagnosed as a myocardial infarction, code the MI type: I21.A1 for type 2 MI from supply and demand mismatch (with the cause coded first), or I21.4 for NSTEMI. Unstable angina without infarction is I20.0. For the full MI code set, see the myocardial infarction guide.
