How to code Dysphagia

R13.10 is dysphagia, unspecified. When the phase is documented, use the specific code: R13.11 oral phase, R13.12 oropharyngeal phase, R13.13 pharyngeal phase, R13.14 pharyngoesophageal phase, and R13.19 for other dysphagia such as esophageal or cervical dysphagia. Aphagia (inability to swallow) is R13.0.

R13.1- carries a code first note for the underlying cause when known. Dysphagia as a late effect of stroke is coded to I69.- with the dysphagia code added to show the phase, for example I69.391 (dysphagia following cerebral infarction) with R13.12. I69.891 and I69.991 cover other and unspecified cerebrovascular disease.

When the cause is a confirmed esophageal condition, that condition is coded, such as K22.2 for esophageal obstruction or stricture and K20.0 for eosinophilic esophagitis. Add the R13.1- code only when the dysphagia is not routinely part of the condition or the documentation supports it.
