How to code Cognitive Impairment
G31.84 is mild cognitive impairment of uncertain or unknown etiology. When MCI is due to a known physiological condition, such as a stroke or Parkinson disease, it is coded as mild neurocognitive disorder F06.70 (without behavioral disturbance) or F06.71 (with behavioral disturbance), with the underlying condition coded first.
R41.81 is age-related cognitive decline (senility not otherwise specified). Specific deficits are coded from R41.84-: R41.840 attention and concentration deficit, R41.841 cognitive communication deficit, R41.842 visuospatial deficit, R41.843 psychomotor deficit, and R41.844 frontal lobe and executive function deficit. R41.89 and R41.9 cover other and unspecified cognitive symptoms.
Cognitive impairment is not the same as dementia. Once the provider diagnoses dementia, use F03.- (unspecified dementia, such as F03.90) or the specific type, for example G30.9 with F02.- for Alzheimer disease. See the dementia guide for the severity and behavior characters.
