T84.04 was in use from FY2016 to FY2016. It was deleted on October 1, 2016 and is not valid for claims with dates of service on or after that date.

CMS did not name a direct replacement. Look under T84.0 (Mechanical complication of internal joint prosthesis) for the current codes.
